Calcitriol is a potent active Vitamin D analogue and must be used with extreme caution. Regular monitoring of serum calcium (every 1–2 weeks initially), serum phosphorus, and renal function is mandatory during therapy. Hypercalcaemia is the primary risk of overdose — early symptoms include anorexia, nausea, vomiting, and muscle weakness, progressing to confusion, polyuria, and cardiac arrhythmias if untreated. Concurrent use of thiazide diuretics significantly increases hypercalcaemia risk. Calcium-containing antacids should not be co-administered without medical supervision. Magnesium-containing antacids should be avoided in renal failure patients as it may lead to hypermagnesaemia. This formulation should not be used in patients with hypercalcaemia, metastatic calcification, or Vitamin D toxicity. Not recommended in pregnancy and lactation without specialist guidance. Patients with granulomatous diseases (sarcoidosis, tuberculosis) should use this with caution as they may be hypersensitive to Vitamin D. Store in a cool, dry place below 25°C, away from direct sunlight. Softgel capsules should not be crushed or chewed.
